Sharpe Care is owned and operated by Heyde Health System, a multi-facility provider of long-term health care in Wisconsin.
Heyde Health System's quality assurance programs ensure that its facilities provide the highest quality of care to each of its residents.
We offer skilled nursing services and assessments to promote the highest level of wellness. Single and Semi-Private rooms are available and residents are encouraged to bring in their own personal items. Each room is furnished with a bed, dresser, chairs, and draperies. Televisions may be brought in and residents may opt to have their own telephone.
Transitional care is provided to individuals who need short term skilled care between the hospital and home. Our professional team will create an individualized care program designed to return each patient home as quickly and safely as possible.
A spacious, beautifully decorated lobby area
Physicians are available to coordinate the quality and delivery of each resident's treatment. Physicians participate in quality assurance meetings and schedule routine visits to direct the resident's care plan while at Sharpe Care.
Our staff is dedicated and compassionate, and provide 24 hour professional nursing service that is committed to each resident's well being. Comprehensive care planning is provided to maximize individual abilities.
Residents are provided with a full range of physical, occupational and speech therapy services. Evaluations and treatments are available seven days a week as ordered by the attending physician.
Our social worker provides a holistic approach to personalized care. A good relationship and communication between staff and residents and the resident's family and friends is the basis of quality care. Social Services coordinates all conferences and services with health care professionals and assists in any discharge planning needs.
Residents are encouraged to bring amenities from home to personalize their rooms.
The dietary department is proficient in meeting all specialized dietary needs and serving nutritious meals and snacks. Our cooks prepare delicious meals using the highest quality food products. Whether it be a special occasion, birthday, holiday or just a daily meal our residents are encouraged to linger and socialize in our spacious dining room.
Our trained staff recognizes residents as individuals and personalizes care to their unique lifestyle, talents, abilities and tolerances. We believe that our residents quality of life can significantly improve by creating a structured lifestyle designed to reduce stress and restore self-worth.
We strive to meet the physical, mental and psychosocial well-being of each resident with creative and interesting daily activities. A facility van provides outings to community events and sightseeing.
